What do you need the EZER for?

Guidance is in English. The step-by-step PDF guides are currently in Dutch.

Route overview.

You can read all routes here without choosing a situation first.

You apply yourself at UWV with the form Voorziening of vergoeding aanvragen on mijn.uwv.nl. This applies if you work or start soon, are re-integrating or applying for jobs, or are self-employed.

J58 and supplier Health2Work cannot submit the application for you, but help with product information and a quote. A medical report follows within 14 days after the application.

The same UWV form applies for every level, from primary school to university, as long as there is an enrolment at the educational institution.

You apply yourself, for you or your child. J58 and Health2Work help with product information and a quote, and a medical report follows within 14 days.

Contact the Wmo desk of your municipality. A consultant discusses your situation with you in a kitchen-table conversation.

The municipality can fund the EZER as care in kind, or as a personal budget (pgb) with which you buy it directly from J58. Each municipality has its own rules, so check the Wmo atlas for yours.
